Knowing how to choose the right contraceptive method at different life stages can be a challenging process. There is no silver bullet, no perfect method, no 100% efficacy or a \u201cone size fits all\u201d approach. The research continues to show wide gaps between perfect and imperfect use (method vs user failure). We know that nearly 50% of users stop or switch methods within one year of starting; discontinuation rates and side effects are estimated to explain 20-30% of unplanned pregnancies.<\/span>\n<p><span style=\"font-weight: 400;\">For <\/span><i><span style=\"font-weight: 400;\">physicians<\/span><\/i><span style=\"font-weight: 400;\">, prescribing birth control is fairly straightforward. Hormonal birth control and IUDs are usually the methods of choice.\u00a0 But for <\/span><i><span style=\"font-weight: 400;\">patients<\/span><\/i><span style=\"font-weight: 400;\">, choosing a method is more of a trade-off because hormonal methods and IUDs often cause side effects that impact health and quality of life. Choosing the right method requires a mindful examination of many factors: age, reproductive goals, past experiences, number of partners, current needs and preferences, attitudes and expectations, knowledge and education, cultural and religious beliefs, physical and mental health, lifestyle, relationship factors, and personal values. Clearly, there are numerous factors to consider.\u00a0 <\/span><\/p>\n<h3 style=\"line-height: 1.09091;\"><span style=\"font-weight: 400;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-4334\" src=\"https:\/\/poriutivit.com\/staging2\/wp-content\/uploads\/2021\/01\/message-for-gynecologists.jpg\" alt=\"A message for gynecologists and medical practitioners \" width=\"300\" height=\"200\" loading=\"lazy\" srcset=\"https:\/\/poriutivit.com\/staging2\/wp-content\/uploads\/2021\/01\/message-for-gynecologists.jpg 300w, https:\/\/poriutivit.com\/staging2\/wp-content\/uploads\/2021\/01\/message-for-gynecologists-200x133.jpg 200w\" sizes=\"(max-width: 300px) 100vw, 300px\" \/><\/span><\/h3>\n<p><span style=\"font-weight: 400;\">For patients, their decision often comes down to settling for the lesser of \u201call evils,\u201d rather than choosing the best method for their body and health.\u00a0 Finding the best fit means having to juggle the benefits of high efficacy with the drawbacks of unpleasant side effects. Since there is no win-win situation for all, the downsides often outweigh the upsides.\u00a0\u00a0<\/span><\/p>\n<h2><span style=\"font-weight: 400;\">Here are 12 steps that Doctors can take<\/span><\/h2>\n<h3>1. Have a more nuanced conversation with patients about the overall differences between contraceptive methods<span style=\"font-weight: 400;\">.<\/span><\/h3>\n<p><span style=\"font-weight: 400;\">After all, each type of method requires a different mindset, priority, value, and skillset: Here\u2019s a way to break them down into four groups.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Methods that require:\u00a0\u00a0\u00a0\u00a0\u00a0<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">No behavioral effort on the part of the user (IUDs and implants).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Low levels of behavioral effort (remembering to take a pill every day or replacing a vaginal ring every month).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Higher levels of effort (diaphragms, spermicides, condoms)\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">User-controlled and 100% responsibility (such as the Fertility Awareness Method (FAM) or other Sympto-Thermal Methods (STM), fertility apps and devices).\u00a0<\/span><\/li>\n<\/ol>\n<h3><b>2. Accept the fact that for many users, statistical efficacy is not the <\/b><b><i>only<\/i><\/b><b> important factor<\/b><b>\u00a0<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">When it comes to prioritizing birth control methods, most physicians view statistical efficacy as the over-riding consideration.\u00a0 They naturally want their patients to enjoy the \u201ceasiest to use\u201d methods (like HBC and IUDs) as these methods confer high efficacy and are relatively \u201chassle-free.\u201c\u00a0 Yet all methods come with a non-monetary \u201cprice tag,\u201d and only over time will a patient\/couple learn whether their choice was good enough or whether the price they paid was \u201ctoo high.\u201d\u00a0\u00a0\u00a0<\/span><\/p>\n<h3>3. Improve communication by asking relevant and personalized opening questions:<\/h3>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u201cMay I ask you some personal questions that will help decide which method might be the \u201cbest fit\u201d for you?\u201d<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u201cWhat methods have you used in the past and why did you stop using them?\u201d<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u00a0\u201cSince every method has its advantages and disadvantages, what is important to you in a birth control method? Then we can narrow down the options that will best meet your needs and expectations\u201d<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u201cAre there methods you are thinking about or would like more information about?\u201d<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u201cHave you made a decision about which method you prefer?\u201d\u00a0<\/span><\/li>\n<\/ul>\n<h3>4. Focus on the client rather than the method<\/h3>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Listen and conduct a dialogue; try putting yourself in their shoes\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Try to gauge the patient\u2019s needs, priorities, and preferences<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Present the diversity of methods without bias; take cues from patient.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Personalize the context<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Acknowledge the patient\u2019s right to autonomy, dignity, respect and her right to informed consent.\u00a0 She wants to share and weigh her options; she wants more control, and to be a proactive partner.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Encourage discussion \u2013 at eye level<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">\u201cOne size doesn\u2019t fit all\u201d\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Remember that a \u2019safe method\u2019 must coincide with \u201dsafe sex\u201d<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Think \u201coutside the (pill) box\u201d<\/span><\/li>\n<\/ul>\n<h3>5. Stay informed<\/h3>\n<p><span style=\"font-weight: 400;\">The world and technology are constantly changing. There are increasing numbers of trends and developments to follow (e.g. more patients are abandoning hormones for apps with lower efficacy).<\/span><\/p>\n<h3><b>6. Be honest<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Be honest with your patients about<\/span><span style=\"font-weight: 400;\"> what you know and don\u2019t know. Before computers, physicians were the primary source of medical information and advice about contraception. Today it is the internet and social media.\u00a0\u00a0<\/span><\/p>\n<h3><b>7. Accept your time constraints<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Physicians have limited time to adequately educate their patients. As such, they cannot be expected to engage in a thorough counseling process. So why not refer your patients to professional and educational resources and websites?<\/span><\/p>\n<h3><b>8. Recognize your biases<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">The goal is to focus on understanding the needs and preferences of the patient. Better to educate and inform rather than use your authority and expertise to convince her to use something that may not be appropriate for her particular needs.<\/span><\/p>\n<h3>9. Encourage patients to take responsibility<\/h3>\n<p><span style=\"font-weight: 400;\">Let them do their homework by reading and collecting information. Women need accurate information about their cycles, their fertility, and all contraceptive method options in order to make informed decisions about their sexual health.\u00a0\u00a0\u00a0<\/span><\/p>\n<h3>10. Reframe the way you think about efficacy statistics and the way you explain them<\/h3>\n<p><span style=\"font-weight: 400;\">Remember to mention the difference between method failure and user failure and that all methods have their advantages and disadvantages. Emphasize that <\/span><i><span style=\"font-weight: 400;\">the best method for someone is the method they will use correctly and consistently<\/span><\/i><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3>11. Your patients want a partnership and shared decision-making<span style=\"font-weight: 400;\">. <\/span><\/h3>\n<p><span style=\"font-weight: 400;\">Be willing to meet them half-way. <\/span><\/p>\n<h3>12. Screen your clients<\/h3>\n<p><span style=\"font-weight: 400;\">If you are a mental health therapist, be sure to <\/span>screen your clients<span style=\"font-weight: 400;\"> by asking if they are taking hormonal methods (associated with anxiety, mood swings, and depression).<\/span><\/p>\n<p><span style=\"font-weight: 400;\">It\u2019s a long way from the clinic to the bedroom.
